What do you need from me to make a medical claim?
You'll need to upload the following documents with your claim
- Discharge summary from the hospital
- Medical report confirming what treatment you received and the reason for the treatment
- Police report
- Receipts for any costs that you’re claiming for
- Bank statements showing what you paid in AUD, if you paid for costs in a foreign currency with a credit or debit card
